What we can deliver in those two or three weeks patients are in our rehab units is constrained by what supply the hospital orders. We measure functional independence at admission and discharge; we rarely look at the metabolic side of what is keeping that number low. If we can use a low-risk, well-studied intervention like creatine to make the same therapy hours more productive, that is a conversation worth having at the institutional level.

The patients I treat the most are older adults recovering from a stroke, a fall, or a major orthopedic surgery. The literature on muscle protein synthesis in this group is clear: their bodies need more help, not less, to rebuild what was lost. Creatine has decades of safety and efficacy data in older adults, and the gap between what the research shows and what reaches our rehab patients is something I want to help close.
